Overview

We’re hiring a Clinical Care Manager to support children and families involved in the foster care system. In this role, you’ll coordinate behavioral health and supportive services, complete required assessments, and ensure members receive timely, appropriate care across a complex system of providers and community resources. This position is remote within Kansas, but requires significant local travel for in-person visits. Key Responsibilities

Conduct member assessments via phone and in-person visits as required In-person visits within 48 hours of a placement change 7-day follow-up/team meetings after placement changes Develop and maintain individualized care plans / service plans Coordinate care across providers, specialists, community resources, and caregivers Provide member/family education on benefits, treatment options, and available resources Document all care management activity thoroughly to meet regulatory and contract requirements Participate in weekly team meetings and collaborate closely with internal and external partners Clinical Care Manager – Foster Care (Behavioral Health)

Required Qualifications

Master’s degree in Behavioral Health or Social Work OR degree from an accredited school of nursing 2–4 years of related experience (care management/case management/behavioral health) One of the following active licenses (Kansas-aligned): Licensed Master’s Behavioral Health Professional (examples: LCSW, LMSW, LMFT, LMHC, LPC ) OR RN with behavioral health experience (per state/program requirements) Experience coordinating services within child welfare/foster care systems (required for success; candidates without this experience will not be considered) Valid driver’s license and ability to travel frequently for member visits Must be authorized to work in the U.S. without sponsorship now or in the future Clinical Care Manager – Foster Care (Behavioral Health) Ideal Candidate Traits Strong communication, high-quality documentation, adaptability, and a solution-focused, mission-driven approach.
