What You'll Do

Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews Develop individualized care plans that address both behavioral and physical health needs Coordinate care with providers, community organizations, and support services Apply advanced clinical judgment to identify and mitigate risk factors impacting member outcomes Perform crisis intervention for members experiencing behavioral health or medical crises and ensure appropriate referrals and follow-up Utilize Motivational Interviewing techniques to engage members and promote sustainable behavior change Coordinate psychosocial wraparound services and connect members with community resources Support cost-effective care through proactive care planning and resource utilization WHAT YOU BRING - REQUIRED Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or a related field Active, unrestricted independent behavioral health license in your state of residence (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC, or equivalent) 3+ years of post-master's direct clinical experience in a hospital, ambulatory, outpatient, or similar clinical setting 3+ years of experience treating mental health and substance use disorders 3+ years of experience utilizing Motivational Interviewing techniques Case Management experience Ability to simultaneously talk on the phone and document in clinical systems Strong technical proficiency and comfort working in a virtual environment

Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
