The role includes psychosocial assessment, therapy, risk review, and collaboration with other healthcare professionals when a client’s needs involve more than one area of support.

Key Responsibilities

Complete psychosocial assessments with clients and, when appropriate, family members Provide therapy and clinical support through telehealth Recognize risk factors and respond according to established clinical procedures Review changes in a client’s condition and overall situation Work with healthcare professionals involved in the client’s care Contribute to care planning when multiple services are involved Maintain clear and complete clinical documentation Follow Oregon LCSW licensing, confidentiality, and professional requirements Master’s degree in Social Work.
