Under the direction of the department manager, the Medical Assistant will be assisting the team in referral management for High-Risk patients and helping to assist with patient needs. Responsibilities/Day in the Life:

• Receive new patient referrals from clients
• Create charts for all new patients
• Input medical information in new chart
• Help Care Coordinators with patient needs as applicable
• Foster relationships with their team to ensure the CareHarmony culture is positive and supportive.
• May be asked to assist in conducting health assessments telephonically with patients
• Role may require a lot of time spent on the phone
• Other duties as applies

Required Education and Experience:

• Medical Assistant certificate must be active and in good standing

Physical Requirements

• This position is sedentary and will require sitting for long periods of time
• This position will require the ability to speak clearly and listen attentively, often by telephone, for an extended period of time
• The position will require the ability to understand, process, and take thorough notes in real-time on telephone conversations
