What You'll Do

As a PTA with Uplift HomeCare, you'll implement physical therapy treatment plans under the direction and supervision of a licensed Physical Therapist, helping patients work toward improved mobility and independence. Your responsibilities will include Providing therapeutic exercises and interventions according to established treatment plans. Helping patients improve strength, balance, mobility, and functional abilities. Monitoring patient responses and reporting progress or changes in condition to the supervising PT. Educating patients and caregivers on prescribed exercises, safe mobility, and fall prevention. Completing accurate, timely, and compliant clinical documentation. Communicating effectively with the supervising Physical Therapist and interdisciplinary care team. Supporting coordinated care and positive patient outcomes. Participating in required in-service education and quality improvement activities.

What We're Looking For

Active Florida Physical Therapist Assistant (PTA) license. Graduation from an accredited Physical Therapist Assistant program. Successful completion of applicable licensing examination requirements. At least one year of home health experience required. Current CPR certification. Strong communication, organizational, and documentation skills. Valid driver's license, current auto insurance, and reliable transportation. Ability to work independently while following the supervising PT's direction. Willingness to travel to patient homes throughout Riverview, Brandon, and surrounding areas. Your Service Area Riverview
