About the Role

Key Responsibilities

Coordinate and manage client care plans remotely, ensuring all needs are addressed effectively. Conduct virtual client assessments and follow-ups to monitor progress and well-being. Facilitate communication between clients, their families, healthcare providers, and support staff through digital channels. Utilize case management software to maintain accurate and up-to-date client records. Provide information and referrals to community resources and support services. Troubleshoot and resolve issues that may arise in the delivery of remote care services. Develop and implement efficient remote support protocols. Ensure compliance with all relevant privacy regulations and organizational policies. Participate in virtual team meetings and contribute to continuous improvement initiatives. Offer empathetic support and guidance to clients navigating complex care needs.

Requirements

A relevant qualification in Social Work, Community Health, Psychology, or a related field. Minimum of 3 years of experience in community support, social work, or care coordination. Proven experience in managing cases and coordinating services. Excellent proficiency with remote work technologies, including video conferencing platforms and case management software. Strong written and verbal communication skills, essential for remote interaction. Exceptional organizational and time-management abilities.
