Job Title :RN Disease Manager - Remote Job ID :76583 Location :USA - Remote What you will be doing :Provides Disease Management (DM) services to Insurance beneficiaries.The DM nurse will collaboratively assess, plan, implement, coordinate, monitor and evaluate the beneficiary's health and psychosocial needs with a focus on empowering the beneficiary to successfully manage their chronic condition.The DM nurse will partner with the interdisciplinary care team to identify appropriate care to prevent complications related to chronic conditions and minimize the risk of potential exacerbation through the use of evidence-based condition-specific education and coaching.This role requires the individual to develop care plans in collaboration with providers, patients, families as appropriate and assesses the effectiveness and outcomes to improve individual health status.What you must have :Current, unrestricted RN licensure to independently work within scope of practice within state of residenceMust be able to receive a favorable Interim and adjudicated final Department of Defense (DoD) background investigationThe ability to obtain multi-state licensure; if a resident in a NCL stateMinimum of three years clinical experienceExperience in case management, care coordination or discharge planning in an acute, ambulatory or specialty disease management program settings in a healthcare system or managed care organization (MCO)Nice to have :Disease Management experience with a managed care organization (MCO)Bachelor's degree other related health fieldKnowledge of government healthcare programs, TRICARE, and Medicare / MedicaidSalary / Rate Range :$39.00 to $41.00 hourly For more information about TEEMA and to consider other career opportunities, please visit our website at www.teemagroup.com.
