Job Description

The Speech-Language Pathologist is responsible for comprehensive assessments in the patient\'s home, including the completion of skilled documentation through Homecare Homebase. Evaluate patients and perform appropriate therapy interventions Develop a comprehensive plan of care and coordinate multi-disciplinary care Perform clinical documentation and visit notes Continued communication with physician, care team members, patient and family Comfortable working in all aspects of the home health setting Implement and maintain SLP protocols Manage patient treatment and outcomes directly Requirements Current Texas Speech-Language Pathology License One year of SLP experience Experience with Homecare Homebase is helpful, but not required Previous Home Health experience is helpful, but not required Current Texas driver\'s license CPR certification Job Type Full-Time Medical Specialty Home Health Schedule Monday to Friday License/Certification BLS Certification (Required) Speech Therapy License (Required) Work Location Multiple locations/On the road EMR Used Homecare Homebase Benefits 401(k) 401(k) matching Dental insurance Flexible schedule Health insurance Paid time off Referral program Travel reimbursement Vision insurance Experience Speech-Language Pathology: 1 year (Required) #J-18808-Ljbffr Experience: 1 year required
